Olympia — The Washington State Department of Health has canceled or suspended the licenses, certifications, or registrations of the following healthcare providers in the state: The department also immediately suspended the eligibility of those who were banned from practicing in other states.

The department’s Health Systems Quality Assurance Division works with its Board, Commissions, and Advisory Board to set licensing standards for more than 80 healthcare professionals (dentists, nurses, counselors, etc.). Information on disciplinary action against physicians and physician assistants can be found at Washington Medical Commission (WMC) website. For questions regarding WMC disciplinary action, media@wmc.wa.gov.

This site contains information about a healthcare provider’s license status, certification expiration and renewal dates, disciplinary action, and copies of legal documents issued after July 1998. Consumers who believe their healthcare provider has acted unprofessional are also encouraged to call and report a complaint.

In August 2022, the Nursing Aid Program suspended the License for Certified Nursing Aid Roberto Zitri Espinoza Vargas (NC60996291). Vargas abused vulnerable adults who were patients in a locked memory care unit at the welfare facility where he worked. In August 2022, the Secretary of Health suspended the License for Certified Medical Assistants Alexandra Lee Crusell-Tupper (CM60361582) Unprofessional conduct. In April 2021, Krussel-Tupper was found guilty of wrongful imprisonment in Grays Harbor Superior Court.

Oregon: In August 2022, Board of Nursing accepted waiver of registered nurse license Grace Ann Koch (RN00073776). Koch had waived his license to the Oregon Board of Nursing after failing to comply with previous Oregon Board of Nursing orders.
